It emphasizes the need for integrated, mechanism-based, and guideline-driven approaches to improve outcomes and quality of life in frail geriatric patients.
Frail older adults represent a growing demographic due to global population aging. Frailty is a multidimensional geriatric syndrome characterized by diminished physiological reserves, increased vulnerability to stressors, and heightened risk of adverse outcomes. Traditional organ-centric models often fall short in addressing the complex needs of this population. Accumulating evidence highlights the critical role of organ-crosstalk adaptation, wherein inter-organ communication and compensatory mechanisms dictate clinical trajectories. A nuanced understanding of these interactions is essential for the development of effective, patient-centered strategies in geriatric medicine.
Frailty affects an estimated 10-20% of community-dwelling adults over 65, with prevalence rising to over 40% in institutionalized settings. The burden is amplified by the frequent coexistence of multimorbidity, polypharmacy, and disability. Epidemiological studies reveal that frail older adults experience higher rates of hospitalization, institutionalization, and mortality. Evidence supports exercise programs, nutritional optimization, and medication review as foundational strategies. Interventions such as ACE inhibitors or GLP-1 agonists may confer organ-protective effects via anti-inflammatory and metabolic pathways. Integrated care models incorporating geriatricians, primary care providers, rehabilitation specialists, and social workers are essential for mitigating polypharmacy, preventing iatrogenic harm, and promoting adaptive organ-crosstalk.
Recent research explores senolytic therapies, mitochondrial-targeted antioxidants, and agents modulating the gut microbiome as means of enhancing organ-crosstalk adaptation. Digital health technologies and remote monitoring enable early detection of multisystem decompensation and individualized intervention. Trials investigating the efficacy of multi-domain interventions combining physical, cognitive, and pharmacological approaches show promise in reversing frailty trajectories and improving resilience.
