This review explores the complexities arising from such clinical scenarios, emphasizing problem prioritization, underlying mechanisms, epidemiology, risk stratification, diagnostic strategies, evidence-based management, and guideline-driven approaches. The article draws on recent research and clinical practice guidelines to provide a framework for navigating multimorbidity, integrating clinical reasoning, and optimizing outcomes for this growing patient demographic.
The prevalence of adults living with multiple chronic diseases is increasing globally, presenting significant challenges in clinical care due to the intricate interplay of disease processes, polypharmacy, and competing health priorities. Managing such patients requires advanced clinical reasoning, judicious prioritization of health issues, and a holistic understanding of patient-centered care. The complexity is further compounded by the risk of fragmented care, potential for adverse drug interactions, and the limitations of disease-specific guidelines in addressing multimorbidity. This article examines the academic and clinical value of CBL in cultivating the skills necessary to prioritize and manage problems in adults with complex chronic conditions.
Multimorbidity is defined as the coexistence of two or more chronic conditions within a single individual, often including common diseases such as hypertension, diabetes mellitus, chronic kidney disease, heart failure, and chronic obstructive pulmonary disease. Epidemiological data indicates that over 60% of adults aged 65 and above have at least two chronic conditions, with the burden disproportionately affecting socioeconomically disadvantaged populations. The growing disease burden is associated with increased healthcare utilization, higher mortality, reduced quality of life, and substantial economic impact.
