PD is primarily characterized by the degeneration of dopaminergic neurons in the substantia nigra, leading to a deficiency of dopamine in the striatum. This imbalance disrupts motor control, causing the hallmark symptoms of PD. The exact cause of neuronal loss is unknown, but genetic factors, environmental exposures, and aging contribute to disease onset.
PD manifests as both motor and non-motor symptoms. Motor symptoms include bradykinesia, rigidity, rest tremor, and postural instability. Non-motor symptoms can encompass autonomic dysfunction, sleep disturbances, cognitive impairment, and mood disorders. These symptoms often precede motor manifestations and can significantly impact a patient's quality of life.
Diagnosis of PD is primarily clinical, based on the presence of cardinal motor symptoms and response to dopaminergic therapy. Neuroimaging, such as DAT-SPECT, can support the diagnosis but is not routinely used. Early diagnosis is paramount for timely intervention and management.
Treatment of PD is individualized, aiming to alleviate symptoms and improve functionality. Pharmacological therapy, primarily levodopa, remains the mainstay of treatment. Adjunct therapies include dopamine agonists, MAO-B inhibitors, and deep brain stimulation. Non-pharmacological approaches, such as physiotherapy and occupational therapy, play a vital role in managing motor and non-motor symptoms.
PD is a complex neurodegenerative disorder that requires a comprehensive understanding for effective management. As healthcare professionals, staying abreast of the latest research and therapeutic strategies is essential to provide the best care for patients living with this challenging condition.
