FHN emphasizes collaborative care, patient empowerment, and the navigation of social, clinical, and systemic barriers, aiming for improved health outcomes and quality of life. This article reviews the scientific foundation, clinical practice, and future prospects of community-oriented family health navigation, drawing from recent PubMed-indexed research and expert guidelines.
