Unexplained weight loss (UWL), defined as a reduction of body weight unintentionally, is a common clinical problem that often poses a diagnostic challenge to physicians. While it can be a symptom of numerous underlying conditions, the key to an accurate diagnosis lies in a comprehensive, systematic approach.
The initial evaluation should involve a detailed patient history and physical examination. Inquiring about the time frame and magnitude of weight loss, associated symptoms, and dietary and lifestyle changes can provide valuable clues. The physical examination should focus on signs of malnutrition or systemic diseases.
Basic laboratory tests can help identify potential causes of UWL. A complete blood count, metabolic panel, thyroid function tests, and urinalysis are usually recommended. Additional tests, such as tumor markers or imaging studies, may be necessary based on the patient's history and physical findings.
Imaging plays a crucial role in diagnosing UWL when the cause is not evident from history, physical examination, and basic laboratory tests. CT scans, MRI, and PET scans can reveal hidden malignancies, infections, or inflammatory conditions that may contribute to weight loss.
If gastrointestinal causes are suspected, endoscopy and biopsies can help identify conditions such as celiac disease, inflammatory bowel disease, or malignancies. However, these procedures should be reserved for patients with specific symptoms or risk factors.
Diagnosing unexplained weight loss requires a comprehensive and systematic approach. Physicians should not only rely on advanced investigations but also give due importance to a detailed history and physical examination. This approach can help identify the underlying cause, allowing for timely intervention and improved patient outcomes.
