Enteric fever, encompassing both typhoid and paratyphoid fever, remains a significant global health issue, particularly in low-resource settings. Despite advancements in diagnostic techniques and treatment strategies, challenges persist in managing this condition. This article aims to provide an updated approach to the diagnosis and management of enteric fever in clinical practice.
Early diagnosis of enteric fever is crucial to prevent complications and reduce morbidity. While blood culture remains the gold standard, it has limitations, such as low sensitivity and slow turnaround time. Recent advances include molecular techniques like PCR and serological tests like Typhidot and Tubex TF, which offer more rapid results. However, their utility is limited by variable sensitivity and specificity. Therefore, a high index of clinical suspicion and comprehensive patient history remain vital in guiding the diagnostic process.
The mainstay of enteric fever management is antibiotic therapy. The choice of antibiotics depends on local antimicrobial resistance patterns. Fluoroquinolones, cephalosporins, and azithromycin are commonly used. However, the emergence of extensively drug-resistant (XDR) strains poses a significant challenge, necessitating the use of newer agents like carbapenems and tigecycline.
Vaccination plays a crucial role in the prevention of enteric fever. Two vaccines, Vi polysaccharide vaccine and Ty21a, are currently available and recommended for high-risk individuals and travelers to endemic areas. Additionally, improving sanitation, safe food and water practices, and public health surveillance are essential in controlling the disease.
Enteric fever continues to pose a significant global health challenge. While advances have been made in diagnostic techniques and treatment strategies, vigilance in clinical suspicion, awareness of local antimicrobial resistance patterns, and preventive measures remain key to managing this disease effectively. Further research is needed to develop more sensitive and specific diagnostic tests and to address the growing concern of antibiotic resistance.
