Unani medicine, an ancient system rooted in Greco-Arabic medical traditions, has increasingly found a complementary role within modern surgical care, particularly in the context of postoperative functional restoration. This article critically reviews the integration of Unani-based approaches in surgical recovery, evaluating clinical evidence, mechanistic rationale, and emerging therapeutic innovations. Emphasis is placed on the practical application of Unani principles, such as the modulation of humoral balance and natural healing processes, alongside contemporary surgical protocols. Drawing from recent research and guideline recommendations, this review aims to inform clinicians of the clinical utility, benefits, and future directions of Unani-based postoperative care.
Surgical advancements have markedly improved patient outcomes, yet postoperative functional restoration remains a challenge, particularly in complex or high-risk surgical populations. Unani medicine, with its holistic and individualized approach, offers a unique perspective on healing and rehabilitation. Historically centered around the balance of four humors—blood, phlegm, yellow bile, and black bile—Unani therapies encompass dietetics, pharmacotherapy, regimental therapies (Ilaj-bil-Tadbeer), and lifestyle modifications. Modern interest has grown regarding their potential role in enhancing recovery, reducing complications, and optimizing functional outcomes post-surgery. This review synthesizes the current evidence base and explores the mechanistic underpinnings and clinical implications of integrating Unani-based concepts into postoperative care.
The global surgical volume exceeds 300 million procedures annually, with postoperative complications contributing significantly to morbidity, prolonged hospitalization, and healthcare costs. Functional deficits following surgery—ranging from impaired mobility to chronic pain and delayed wound healing—are prevalent, especially in aging populations and those with comorbidities. Conventional rehabilitation strategies, while effective, are not universally accessible or tolerated. The integration of complementary systems, such as Unani medicine, is emerging as a strategy to address unmet needs and promote holistic recovery, particularly in regions where Unani practice is established, such as South Asia, the Middle East, and North Africa.
Postoperative functional impairment is multifactorial, involving tissue injury, inflammation, oxidative stress, neuroendocrine dysregulation, and immobility. Unani medicine attributes disease and delayed healing to humoral imbalance, with surgical trauma considered a disruptor of the natural equilibrium. Unani regimens aim to restore balance through detoxification, modulation of immune and inflammatory responses, and supporting tissue regeneration. Key concepts include the use of Munzij (detoxifying agents), Mushil (purgatives), and Mufarreh (tonics), which are believed to enhance circulation, remove toxic byproducts, and promote healing at the cellular and systemic levels. This mechanistic overlap with modern understandings of wound healing and immunomodulation provides a scientific rationale for Unani interventions in postoperative care.
Several factors predispose patients to suboptimal postoperative function, including advanced age, diabetes, obesity, nutritional deficiencies, pre-existing functional impairment, and the magnitude of surgical insult. From a Unani perspective, risk stratification also considers temperament (Mizaj), humoral status, and lifestyle factors. These determinants guide individualized therapeutic strategies, aiming to preempt complications and tailor regimens to the patient's physiological and constitutional needs.
Patients experiencing postoperative functional decline may present with fatigue, reduced mobility, persistent pain, delayed wound healing, and psychological distress. Unani practitioners assess not only the physical manifestations but also systemic symptoms such as sleep disturbances, gastrointestinal dysfunction, and mood alterations, reflecting a comprehensive approach to recovery. The identification of subtle signs of humoral imbalance can facilitate early intervention and more targeted restoration strategies.
Standard postoperative assessment involves clinical evaluation, functional scoring systems, laboratory investigations, and imaging. Unani diagnosis integrates these modalities with traditional methods, including pulse examination, urine analysis, and detailed history-taking to assess humoral status and temperament. The integration of both paradigms enhances diagnostic accuracy and informs the selection of complementary therapies for optimal functional recovery.
Conventional postoperative care prioritizes early mobilization, pain control, nutritional support, and physical rehabilitation. Unani-based management augments these protocols with interventions such as personalized dietary plans (Ilaj-bil-Ghiza), herbal formulations (Ilaj-bil-Dawa), and regimental therapies like massage (Dalak), cupping (Hijama), and steam therapy (Hammam). Clinical studies have reported benefits of Unani interventions in improving wound healing, reducing inflammation, and enhancing overall functional outcomes. Integration is most effective when Unani therapies are employed as adjuncts, ensuring safety and compatibility with allopathic care.
Recent research has focused on the bioactive properties of Unani medicinal plants, such as Nigella sativa, Withania somnifera, and Aloe vera, demonstrating anti-inflammatory, antioxidant, and regenerative effects relevant to postoperative healing. Randomized controlled trials and observational studies are increasingly evaluating Unani regimens in surgical settings, with promising results in pain reduction, faster return to function, and decreased incidence of complications such as surgical site infections. Emerging therapies also include the use of nano-formulations of traditional Unani compounds for enhanced bioavailability and targeted action. Advances in personalized medicine and integrative care pathways are facilitating the systematic inclusion of Unani principles in perioperative protocols.
International and national guidelines increasingly recognize the value of integrative medicine in surgical recovery, emphasizing evidence-based complementary therapies with established safety profiles. The Ministry of AYUSH (India) has issued recommendations for the integrative use of Unani medicine in postoperative care, highlighting the importance of practitioner training, patient education, and rigorous monitoring. Multidisciplinary collaboration is endorsed to optimize outcomes and mitigate risks, with emphasis on individualized care plans and ongoing evaluation of efficacy and safety.
The incorporation of Unani-based concepts into postoperative functional restoration represents a promising paradigm shift in surgical care. By addressing the multifaceted needs of recovering patients through a blend of traditional wisdom and modern evidence, Unani medicine can enhance functional outcomes, reduce complications, and support holistic healing. Ongoing research, interdisciplinary collaboration, and guideline-driven practice are essential to fully realize the potential of Unani interventions in the surgical setting, ensuring patient-centered and scientifically rigorous care.
