The integration of household resilience-centered primary healthcare (PHC) models represents a transformative approach to improving quality of life (QoL), particularly in resource-limited and variable socio-economic contexts. This review synthesizes recent research, clinical guidelines, and implementation experiences to highlight the epidemiology, underlying mechanisms, risk factors, clinical features, diagnostic approaches, management strategies, and emerging therapies associated with these models. It further explores guideline recommendations, drawing evidence-based conclusions to inform clinicians and healthcare policymakers about the practical and clinical implications of adopting household resilience as a central tenet of PHC delivery.
Quality of life is a multidimensional construct encompassing physical, mental, and social well-being. Traditional PHC models often center on individual patients, sometimes neglecting the household as a dynamic unit of health and resilience. Household resilience-centered PHC models prioritize the capacity of families to anticipate, cope with, recover from, and adapt to health-related adversities. This paradigm shift aligns with the World Health Organization’s (WHO) vision for people-centered care and leverages the household as the locus for health promotion, disease prevention, and chronic disease management. Recent evidence suggests that such models can yield substantive improvements in health outcomes, patient satisfaction, and system sustainability by addressing social determinants of health and fostering adaptive capacities at the micro-community level.
Globally, chronic diseases, mental health disorders, and acute infections continue to impose significant morbidity and mortality burdens. The COVID-19 pandemic underscored vulnerabilities in traditional PHC systems, especially in underserved communities. Households with low resilience face disproportionate impacts from health crises, exacerbating inequities in outcomes. Epidemiological studies, including multicountry analyses and community health assessments, demonstrate that interventions which enhance household resilience—such as health literacy programs, economic empowerment, and social support networks—correlate with reduced hospitalizations, improved chronic disease control, and superior QoL indices. The burden is particularly acute in low- and middle-income countries, where social determinants play an outsized role in shaping health trajectories.
Household resilience influences pathophysiological processes through mechanisms involving stress modulation, behavioral adaptation, and resource mobilization. Chronic stress due to economic insecurity, environmental hazards, or familial instability activates neuroendocrine pathways, inducing allostatic load and predisposing to metabolic, cardiovascular, and mental health disorders. Conversely, resilient households deploy adaptive coping strategies—such as mutual support, shared health-seeking behaviors, and collective problem-solving—that mitigate stress-induced pathologies and foster protective health behaviors. The interplay between household environment and individual biology underscores the necessity of integrating resilience-building interventions into PHC frameworks.
Key risk factors for diminished household resilience and poor QoL include socioeconomic deprivation, unstable housing, limited access to healthcare, low health literacy, and fragmented social networks. Additional contributors encompass chronic disease burden within the home, caregiving responsibilities, and exposure to violence or environmental hazards. Vulnerable subpopulations—such as single-parent families, elderly caregivers, and households with disabled members—require particular attention. Understanding these risk factors is critical for tailoring PHC interventions that promote resilience and address the root causes of health disparities.
Clinically, households with low resilience often present with complex, interrelated health issues: recurrent acute illnesses, poorly controlled chronic diseases, psychosomatic complaints, and elevated rates of mental health conditions. Social isolation, caregiver burnout, and non-adherence to medical regimens are prevailing features. Conversely, resilient households tend to demonstrate proactive health behaviors, robust support systems, and higher engagement with preventive services. Recognizing these clinical patterns allows for early identification and targeted intervention within the PHC setting.
Assessment of household resilience is inherently multidimensional, requiring tools that capture social, psychological, and functional domains. Standardized instruments such as the Household Resilience Index and Family Resilience Assessment Scale facilitate systematic evaluation. Comprehensive PHC assessments now increasingly incorporate questions about resource availability, coping strategies, social support, and environmental stability. Integrating these findings into the clinical workflow supports individualized care planning and enhances the precision of interventions.
Management strategies focus on empowering households through health education, self-management support, and facilitation of access to social and economic resources. Multidisciplinary PHC teams—including nurses, social workers, and community health workers—play a central role in delivering coordinated care. Interventions such as family counseling, group medical visits, and community-based rehabilitation have demonstrated efficacy in enhancing resilience and improving QoL. Emphasizing culturally sensitive approaches and active patient-family engagement further optimizes outcomes.
Recent advances include digital health platforms that enable virtual family consultations, remote monitoring, and telehealth-based resilience coaching. Emerging therapies leverage behavioral economics, motivational interviewing, and peer-support models to drive sustained behavior change. Population health management systems now integrate social determinants data to stratify risk and allocate resources more effectively. Pilot programs in various settings have shown promise, with improved medication adherence, reduced emergency visits, and higher patient-reported QoL scores.
Leading organizations—including the WHO and national primary care bodies—now recommend routine assessment of household context and resilience in PHC practice. Guidelines advocate for integrated care pathways that address medical, social, and behavioral determinants, supported by intersectoral collaboration. Universal screening for social needs, structured family interventions, and ongoing professional education are emphasized as cornerstones of effective PHC delivery. Quality improvement initiatives should prioritize patient and family engagement, continuous monitoring, and iterative adaptation of care models based on real-world outcomes.
Resilience-centered household PHC models offer a robust, evidence-based framework for enhancing quality of life and reducing health disparities. By shifting the unit of care from the individual to the household, clinicians can more effectively address the complex interplay of biological, social, and environmental determinants of health. Continued research, implementation science, and policy innovation are required to optimize these models and ensure their scalability and sustainability in diverse healthcare settings.
