Fertility preservation is a rapidly evolving field with significant implications for individuals at risk of compromised reproductive potential. This review explores the current landscape of fertility preservation awareness within reproductive health education, emphasizing the importance of early intervention, evidence-based strategies, and guideline-driven practice. Focusing on the epidemiology, mechanisms, risk factors, clinical manifestations, diagnosis, management, and emerging advances, this article provides a comprehensive resource for clinicians aiming to integrate fertility preservation counseling into routine care. The review also discusses recent guideline recommendations to optimize patient outcomes and highlights the critical need for enhanced education and awareness among healthcare professionals and patients alike.

Introduction

Fertility preservation has become a cornerstone of reproductive medicine, particularly for individuals facing gonadotoxic treatments or conditions threatening future fertility. The expanding indications for fertility preservation, including oncologic, genetic, autoimmune, and iatrogenic causes, necessitate heightened awareness and integration within reproductive health education. Despite advancements in assisted reproductive technologies (ART) and preservation techniques, knowledge gaps persist among healthcare providers, potentially leading to missed opportunities for timely intervention. This review aims to synthesize current evidence, highlight clinical implications, and guide practitioners in delivering comprehensive, patient-centered fertility preservation care.

Epidemiology / Disease Burden

The prevalence of conditions necessitating fertility preservation is rising, reflecting broader trends in cancer survivorship, delayed childbearing, and improved diagnosis of reproductive disorders. According to recent population-based studies, the incidence of cancer in individuals of reproductive age remains substantial, with up to 15% of newly diagnosed malignancies occurring before the age of 40. Additionally, autoimmune diseases requiring cytotoxic therapy and genetic syndromes associated with premature gonadal failure contribute to the burden. Additional considerations include the presence of comorbidities such as endometriosis, polycystic ovary syndrome (PCOS), and autoimmune diseases, which may independently compromise fertility. Emerging evidence also implicates lifestyle factors—such as smoking, obesity, and environmental exposures—in modulating reproductive risk. Comprehensive risk assessment is vital to identify candidates who may benefit most from fertility preservation interventions.

Clinical Features

Patients at risk for fertility impairment may present with a spectrum of clinical features depending on the underlying etiology. In oncologic populations, the need for urgent cancer therapy often precludes detailed reproductive assessment; however, menstrual irregularity, amenorrhea, decreased testicular volume, or changes in secondary sexual characteristics may signal impending gonadal failure. In non-oncologic scenarios, insidious decline in ovarian reserve may be detected via diminished anti-Müllerian hormone (AMH) levels or altered menstrual patterns. Prompt recognition of clinical features is critical, as early intervention maximizes the efficacy of preservation strategies and aligns with patient reproductive goals.

Diagnosis

Diagnostic evaluation for fertility preservation candidates encompasses a thorough medical, reproductive, and family history, hormonal profiling, and imaging. Baseline assessment includes serum AMH, follicle-stimulating hormone (FSH), estradiol, and antral follicle count (AFC) in females; and semen analysis, FSH, luteinizing hormone (LH), and inhibin B in males. Advanced modalities, such as ovarian reserve testing and testicular biopsy, may be indicated in select cases. Timely and accurate diagnosis ensures individualized preservation planning and informs counseling regarding prognosis and available options.

Treatment & Management

Fertility preservation encompasses a range of interventions tailored to individual circumstances. Oocyte and embryo cryopreservation represent the standard of care for females, while sperm cryopreservation is the gold standard for males. Ovarian tissue cryopreservation, once considered experimental, is now endorsed for prepubertal girls and women who cannot delay cancer therapy. Testicular tissue cryopreservation is an emerging option for prepubertal boys. Adjuvant treatments, such as ovarian suppression with GnRH analogs, may offer additional protection. Multidisciplinary collaboration between oncologists, reproductive endocrinologists, and allied health professionals is essential for comprehensive management.

Significant advances have expanded the landscape of fertility preservation. Innovations in vitrification have improved oocyte and embryo survival rates, while advances in in vitro maturation (IVM) and artificial gametogenesis hold promise for patients with limited preservation windows. Ovarian tissue transplantation has resulted in successful pregnancies, and research into stem cell-based therapies may further revolutionize the field. Genetic screening and personalized medicine approaches are increasingly integrated into preservation decision-making. These advances underscore the importance of ongoing education and research to optimize patient care.

Guideline Recommendations

Multiple specialty societies, including the American Society for Reproductive Medicine (ASRM) and the European Society of Human Reproduction and Embryology (ESHRE), have issued evidence-based guidelines emphasizing early counseling and timely referral for fertility preservation. Key recommendations include routine fertility risk assessment in at-risk populations, prompt initiation of preservation strategies prior to gonadotoxic therapy, and multidisciplinary care coordination. Guidelines also highlight the importance of patient-centered counseling that addresses the psychosocial, ethical, and financial implications of fertility preservation. Ongoing professional education is recommended to ensure guideline adherence and optimal patient outcomes.
