Introduction

Enteric fever, predominantly caused by Salmonella Typhi and Paratyphi, remains a significant health issue worldwide, particularly in developing nations. This article aims to provide an updated approach to its diagnosis and treatment, based on recent research and advancements.

Diagnosis

Early diagnosis of enteric fever is crucial for effective management. Traditionally, blood culture has been the gold standard for diagnosis. However, its sensitivity is not optimal, especially in early disease stages or after antibiotic therapy. Recent advancements include nucleic acid amplification tests (NAATs), which have shown promising results in terms of sensitivity and specificity. Moreover, serological tests like Typhidot and Tubex have improved, although their reliability varies across different populations.

Treatment

Treatment of enteric fever mainly involves antibiotic therapy. The choice of antibiotics has evolved due to increasing antibiotic resistance. Fluoroquinolones, once the first-line treatment, have seen decreased efficacy due to widespread resistance. Current guidelines recommend third-generation cephalosporins like ceftriaxone or cefixime, or azithromycin as first-line treatment. However, the choice should be guided by local susceptibility patterns. Moreover, supportive care including hydration and fever control is vital.

Prevention and Control

Prevention plays a key role in managing enteric fever. This includes provision of safe drinking water, improved sanitation, and vaccination. Two vaccines, Vi polysaccharide vaccine and Ty21a, are currently available. However, their use is recommended in high-risk areas or for travelers to such areas, as their efficacy is not 100%.

Conclusion

Enteric fever continues to pose a significant health challenge, especially in resource-limited settings. Early diagnosis and appropriate treatment are essential for patient recovery. The emergence of antibiotic resistance necessitates continuous surveillance and updating of treatment guidelines. Preventive strategies, including vaccination and improvements in water and sanitation, are equally important in controlling the disease.
