Abstract

Survivorship science in pediatric oncology has evolved dramatically over recent decades, driven by increasing cure rates and a growing population of childhood cancer survivors. This review synthesizes current knowledge on the epidemiology, pathophysiology, risk factors, clinical features, diagnostic approaches, management strategies, and emerging therapies relevant to long-term survivorship in pediatric cancer. Emphasis is placed on evidence-based findings, mechanistic insights, and practical clinical implications to inform multidisciplinary care and optimize lifelong health outcomes for survivors.

Introduction

With advancements in diagnostics, therapeutics, and supportive care, survival rates for childhood cancers have improved significantly, leading to a burgeoning cohort of pediatric cancer survivors who require ongoing multidisciplinary care. Survivorship science now encompasses a broad spectrum of research and clinical practice, focusing on mitigating late effects, enhancing quality of life, and developing individualized follow-up strategies. This article reviews the most recent evidence and clinical guidelines informing pediatric long-term survivorship, providing a comprehensive resource for healthcare professionals.

Epidemiology / Disease Burden

Globally, childhood cancers constitute approximately 1% of all new cancer diagnoses, with incidence rates varying by region and cancer type. Subclinical organ dysfunction is common, necessitating proactive surveillance and early intervention to prevent progression to overt disease.

Diagnosis

Diagnosis of late effects relies on structured long-term follow-up protocols incorporating a combination of clinical assessments, laboratory investigations, and imaging modalities tailored to individual risk profiles. Consensus guidelines, such as those from the Children's Oncology Group (COG), recommend risk-adapted screening for cardiac, endocrine, neurocognitive, and secondary cancer risks. Emerging biomarkers and advanced imaging techniques (e.g., cardiac MRI, neuroimaging) are enhancing sensitivity and specificity in the early detection of subclinical sequelae, facilitating timely interventions.

Treatment & Management

Comprehensive survivorship care integrates preventive, therapeutic, and rehabilitative strategies, delivered through multidisciplinary teams. Management of late effects includes evidence-based pharmacologic interventions (e.g., ACE inhibitors for cardiomyopathy, hormone replacement for endocrine disorders), psychosocial support, lifestyle modification, and tailored rehabilitation programs. Transitioning from pediatric to adult care is a critical juncture requiring structured education, coordinated handoffs, and ongoing risk-based surveillance to ensure continuity of care and adherence to recommended screening protocols.

Recent Advances / Emerging Therapies

Recent advances in survivorship science include the development of precision medicine approaches incorporating genetic risk stratification, implementation of digital health tools for remote monitoring, and novel therapeutics reducing late treatment-related toxicities. CAR-T cell therapies and targeted agents (e.g., tyrosine kinase inhibitors) are demonstrating efficacy with potentially lower long-term morbidity compared to conventional modalities. Research into interventions to promote organ regeneration, neuroprotection, and immune modulation holds promise for further reducing late effects and improving quality of life.

Guideline Recommendations

Leading organizations, including the COG, American Society of Clinical Oncology (ASCO), and International Late Effects of Childhood Cancer Guideline Harmonization Group, have developed risk- and exposure-based guidelines for survivorship care. These recommend individualized long-term follow-up, regular risk-based screening, psychosocial assessment, and patient education. Integrating survivorship care plans into electronic health records and enhancing provider and survivor education are key strategies for optimizing adherence and outcomes.
