Syncope, a transient loss of consciousness due to global cerebral hypoperfusion, is a medical condition that often presents a diagnostic challenge. This article aims to provide healthcare professionals with a comprehensive approach to understanding, diagnosing, and managing syncope.
Syncope is a symptom, not a diagnosis, and its underlying causes can be diverse, ranging from benign to life-threatening. It accounts for approximately 1% of all emergency department visits. The common causes are neurocardiogenic, orthostatic hypotension, and cardiac syncope. However, about 50% of the time, the cause remains unknown, termed as unexplained syncope.
The diagnosis of syncope begins with a detailed history and physical examination. The goal is to identify any potential triggers, prodromal symptoms, and the circumstances surrounding the event. Special attention should be given to cardiac history. Diagnostic tests such as electrocardiogram (ECG), Holter monitoring, echocardiography, and tilt-table testing can be used based on clinical suspicion. However, over-reliance on diagnostic tests without a clear clinical indication can lead to unnecessary costs and potential harm to patients.
The management of syncope is primarily directed towards the underlying cause. In cases of unexplained syncope, the focus should be on risk stratification and patient education. Non-pharmacological interventions such as physical counter-pressure manoeuvres can be effective. Pharmacological treatment options are limited and should be considered only when non-pharmacological interventions are ineffective or not feasible. In patients with high-risk features, more invasive strategies such as pacemaker or implantable cardioverter-defibrillator (ICD) may be considered.
Understanding, diagnosing, and managing syncope requires a comprehensive, patient-centred approach. While the diagnostic journey can be challenging, a systematic approach can help in identifying the underlying cause and appropriate management. Ongoing research in this field promises to shed more light on the enigmatic nature of syncope and improve patient outcomes.
