Abstract

Cardiac sarcoidosis (CS), a manifestation of systemic sarcoidosis, presents significant diagnostic and management challenges due to its heterogeneous clinical spectrum and frequent multisystem involvement. This review synthesizes recent evidence, guidelines, and expert consensus on the case-based management of CS, emphasizing a multidisciplinary approach, mechanism-based interventions, and advances in diagnosis and therapy. Clinicians must recognize the complex interplay between cardiac and extracardiac manifestations to optimize patient outcomes.

Introduction

Sarcoidosis is a systemic granulomatous disease of unknown etiology, characterized by non-caseating granulomas that can affect virtually any organ. Cardiac involvement, while less common than pulmonary or lymphatic disease, carries a high risk of morbidity and mortality, primarily due to arrhythmias, heart failure, and sudden cardiac death. The clinical management of CS is further complicated by its frequent association with multisystem sarcoidosis, necessitating comprehensive and individualized care plans. This review provides an in-depth exploration of case-based strategies for the management of cardiac sarcoidosis with multisystem involvement, guided by the latest research and expert consensus.

Epidemiology / Disease Burden

The true prevalence of cardiac sarcoidosis remains elusive, given the subclinical nature in many cases and variability in diagnostic criteria. Autopsy studies suggest cardiac involvement in approximately 25–30% of patients with systemic sarcoidosis, while clinical detection rates remain lower (5–10%), often due to under-recognition or limitations in diagnostic modalities. Cardiac sarcoidosis exhibits a wide spectrum of clinical manifestations, ranging from asymptomatic disease detected incidentally to life-threatening arrhythmias and heart failure. Presentations include palpitations, syncope, presyncope, chest pain, progressive dyspnea, and sudden cardiac arrest. Conduction abnormalities, such as complete heart block and bundle branch block, are common. Ventricular arrhythmias may manifest as non-sustained or sustained ventricular tachycardia. Heart failure typically results from granulomatous infiltration and subsequent fibrotic remodeling. Multisystem involvement may present concurrently or sequentially, further complicating the clinical picture and necessitating a high index of suspicion in sarcoidosis patients with new cardiac symptoms.

Diagnosis

Diagnostic evaluation of cardiac sarcoidosis requires a high degree of clinical suspicion, especially in patients with known systemic sarcoidosis who develop cardiac symptoms. The Heart Rhythm Society (HRS) and Japanese Ministry of Health and Welfare (JMHW) criteria guide diagnosis, combining clinical, imaging, and histopathologic findings. Advanced imaging modalities, such as cardiac magnetic resonance imaging (CMR) with late gadolinium enhancement and fluorodeoxyglucose positron emission tomography (FDG-PET), have improved sensitivity for detecting myocardial inflammation and scar. Endomyocardial biopsy remains the gold standard but is limited by low sensitivity due to patchy myocardial involvement. Electrocardiography, ambulatory rhythm monitoring, and echocardiography are essential adjuncts for arrhythmia and structural assessment. Multisystem evaluation includes chest imaging, pulmonary function testing, ophthalmological assessment, and laboratory markers (e.g., ACE, sIL-2R).

Treatment & Management

The cornerstone of cardiac sarcoidosis management is immunosuppressive therapy, primarily corticosteroids. Early initiation of steroids (prednisone 30–60 mg/day) is associated with improved arrhythmia control and ventricular function. Steroid-sparing agents such as methotrexate, azathioprine, or mycophenolate mofetil are frequently employed in refractory cases or to minimize steroid burden. Tumor necrosis factor-alpha (TNF-α) inhibitors, including infliximab and adalimumab, have shown efficacy in select steroid-refractory or intolerant cases, particularly with multisystem involvement. Cardiac-specific interventions depend on clinical presentation: pacemaker or implantable cardioverter-defibrillator (ICD) implantation for high-grade heart block or malignant arrhythmias, antiarrhythmic drugs, and conventional heart failure management as per guidelines. Multidisciplinary collaboration is vital for optimal outcomes, necessitating coordinated care among cardiologists, pulmonologists, rheumatologists, and electrophysiologists.

Recent Advances / Emerging Therapies

Recent advances in the management of cardiac sarcoidosis include the development of novel imaging biomarkers, refined risk stratification tools, and emerging immunomodulatory therapies. Cardiac MRI and FDG-PET continue to evolve, providing quantitative metrics for disease activity and therapeutic response. The introduction of biologic agents targeting specific immune pathways (e.g., TNF-α inhibitors) has expanded therapeutic options, particularly for refractory or multisystem disease. Ongoing clinical trials are evaluating agents such as rituximab and other targeted immunomodulators. Advances in device therapy, including subcutaneous and leadless ICDs, offer new options for arrhythmia management in complex cases. Artificial intelligence and machine learning applications hold promise for early detection and personalized risk prediction.

Guideline Recommendations

Expert society guidelines emphasize a tailored, multidisciplinary approach to the management of cardiac sarcoidosis. The HRS recommends immunosuppression as first-line therapy, with escalation to steroid-sparing or biologic agents for refractory disease. Routine screening for cardiac involvement in all sarcoidosis patients with concerning symptoms or high-risk features is advocated. Device therapy should be considered in patients with documented ventricular arrhythmias, significant conduction disease, or reduced left ventricular ejection fraction (<35–40%). Long-term monitoring for disease progression and recurrence is essential, as is coordination of care among relevant specialties. Individualized risk assessment and patient-centered decision-making remain paramount.

Cardiac sarcoidosis with multisystem involvement represents a clinical entity of substantial complexity, requiring nuanced diagnostic acumen and a collaborative, mechanism-based management strategy. Advances in imaging, immunotherapy, and device technologies have transformed the diagnostic and therapeutic landscape, yet challenges remain in optimizing long-term outcomes. Ongoing research and multidisciplinary care are essential to further refine management algorithms and improve the prognosis for patients with this formidable condition.
