Abstract

Hemorrhagic shock remains a leading cause of preventable mortality in trauma and emergency medicine. Recent years have witnessed significant advancements in targeted endovascular hemorrhage control, offering novel minimally invasive techniques to rapidly address life-threatening bleeding. This review synthesizes current evidence, mechanistic insights, and clinical recommendations pertaining to the deployment of targeted endovascular modalities in emergency care settings, with an emphasis on their epidemiological impact, pathophysiological rationale, diagnostic integration, and evolving guideline recommendations.

Introduction

Uncontrolled hemorrhage is a critical challenge encountered in emergency and trauma care, often dictating patient outcomes within the first hours of presentation. Traditional surgical interventions, while effective in selected scenarios, can be limited by time constraints, anatomical considerations, and patient instability. The evolution of targeted endovascular hemorrhage control techniques—including resuscitative endovascular balloon occlusion of the aorta (REBOA), selective arterial embolization, and rapid deployment stent-grafts—has redefined the armamentarium available to emergency clinicians. Understanding the clinical indications, mechanisms, and practical considerations for these therapies is essential for optimizing acute care delivery and improving survival rates.

Computed tomography angiography (CTA) is the gold standard for localizing vascular injuries amenable to endovascular intervention. Laboratory evaluation, including serial hematocrit, lactate, and coagulation studies, aids in quantifying blood loss and guiding resuscitation strategy.

Treatment & Management

Initial management of hemorrhagic shock follows standard Advanced Trauma Life Support (ATLS) algorithms emphasizing airway protection, breathing support, and circulatory stabilization. Targeted endovascular techniques are increasingly integrated into early management algorithms, particularly for NCTH. REBOA involves the percutaneous insertion of a balloon catheter into the aorta, providing proximal vascular control and augmenting cerebral and coronary perfusion. Selective arterial embolization is used to occlude bleeding vessels in solid organ injuries or pelvic fractures. Rapid deployment stent-grafts may be considered for major arterial injuries where vessel preservation is desirable. These modalities are typically coordinated by multidisciplinary trauma and interventional radiology teams.

Recent Advances / Emerging Therapies

Technological and procedural innovations have expanded the scope and safety of endovascular hemorrhage control. Next-generation REBOA catheters permit low-profile insertion and partial occlusion, allowing for controlled reperfusion and reduced ischemic complications. Drug-eluting embolic agents and resorbable materials are under investigation to further minimize re-bleeding risk and downstream tissue damage. Portable fluoroscopy and hybrid operating suites have enhanced the feasibility of endovascular approaches in austere or pre-hospital environments. Ongoing clinical trials are evaluating the efficacy of novel devices and algorithms in both civilian and military trauma populations.

Guideline Recommendations

Contemporary guidelines from trauma societies and emergency medicine organizations increasingly endorse the use of targeted endovascular interventions for select patients with non-compressible hemorrhage. Indications for REBOA include profound hypotension unresponsive to resuscitation and evidence of ongoing intra-abdominal or pelvic bleeding without immediate surgical access. Protocols emphasize the need for appropriate training, multidisciplinary collaboration, and integration with massive transfusion strategies. The importance of timely transition to definitive surgical or radiologic intervention is also underscored to prevent complications associated with prolonged vascular occlusion.

Targeted endovascular hemorrhage control represents a paradigm shift in the acute management of life-threatening bleeding, particularly in the context of non-compressible injuries. Evidence supports the judicious use of these minimally invasive techniques as adjuncts to traditional resuscitative measures, with ongoing innovation promising to broaden their applicability and safety profile. Future directions include expanded pre-hospital deployment, refinement of device technology, and integration of real-time decision support to maximize patient outcomes. Continued research, guideline development, and clinician training will be essential to realize the full potential of these transformative interventions in emergency care.
