Psoriasis and melasma are two chronic dermatological conditions that significantly impact patient quality of life and represent a persistent therapeutic challenge. While topical and procedural interventions dominate initial management, oral therapies play an increasingly pivotal role, especially in moderate-to-severe disease and recalcitrant cases. This review critically examines the current landscape and future directions of oral treatment strategies for psoriasis and melasma, highlighting recent advances, mechanisms of action, practical clinical considerations, and guideline-driven recommendations for healthcare professionals.
Psoriasis and melasma are prevalent skin disorders with distinct etiologies but overlapping challenges in chronicity, relapse, and patient dissatisfaction. Oral therapies, though traditionally reserved for severe cases, have evolved in their utility and scope. With the advent of novel agents and deeper understanding of disease mechanisms, oral treatments are being re-evaluated for their efficacy, safety, and role in comprehensive management. This article provides an in-depth analysis for clinicians seeking up-to-date, evidence-based guidance on oral therapies in these conditions.
Psoriasis affects approximately 2-3% of the global population, with significant morbidity due to its systemic inflammatory nature and association with comorbidities such as psoriatic arthritis, cardiovascular disease, and metabolic syndrome. Melasma, while not associated with systemic disease, is a leading cause of facial hyperpigmentation worldwide, particularly among women of reproductive age and individuals with darker skin phototypes. Both disorders lead to considerable psychosocial distress, reduced productivity, and increased healthcare utilization, underscoring the need for effective, sustainable oral therapies.
Psoriasis is characterized by immune dysregulation, particularly involving Th17 and Th1 pathways, leading to keratinocyte hyperproliferation and chronic inflammation. Key cytokines include TNF-α, IL-17, and IL-23. In contrast, melasma results from complex interplay between genetic predisposition, ultraviolet (UV) exposure, hormonal influences, and vascular factors, culminating in melanocyte hyperactivity and aberrant melanin deposition. Recent studies implicate oxidative stress and dermal inflammation as contributory mechanisms in melasma, expanding the rationale for systemic intervention.
Risk factors for psoriasis include genetic susceptibility (notably HLA-Cw6), environmental triggers (infections, trauma, stress), and lifestyle factors such as obesity and smoking. Melasma risk is heightened by UV exposure, hormonal fluctuations (e.g., pregnancy, oral contraceptives), family history, and certain medications (e.g., antiepileptics). Recognizing these factors is essential for tailored therapeutic strategies and patient counseling regarding oral treatments.
Psoriasis manifests as well-demarcated erythematous plaques with silvery scale, often involving the scalp, elbows, knees, and trunk. Variants include guttate, pustular, and erythrodermic forms. Melasma presents as symmetric, hyperpigmented macules and patches on the face, commonly affecting the cheeks, forehead, and upper lip. Both conditions are chronic, subject to relapse, and can be refractory to topical monotherapy, necessitating consideration of oral agents.
Clinical diagnosis of psoriasis is typically straightforward, supported by characteristic morphology and distribution. Histopathology may be warranted in atypical cases. Melasma is diagnosed clinically, often aided by Wood\'s lamp examination to assess depth of pigmentation. Dermoscopy and reflectance confocal microscopy are adjunctive tools. Laboratory evaluation is necessary prior to initiating certain oral therapies, including baseline liver and renal function tests, and screening for latent infections.
Oral therapies for psoriasis encompass traditional systemic agents (methotrexate, cyclosporine, acitretin), small molecules (apremilast), and, increasingly, JAK inhibitors. These agents exert immunomodulatory effects, targeting key pathways in psoriatic inflammation. In melasma, oral agents are adjunctive to topical and procedural treatments. Tranexamic acid has garnered attention for its anti-plasmin activity and efficacy in reducing melanogenesis. Other agents under investigation include oral antioxidants, polypodium leucotomos, and melatonin. The choice of oral therapy is guided by disease severity, comorbidities, reproductive considerations, and patient preference.
Recent years have witnessed remarkable progress in targeted oral therapies. In psoriasis, the approval of TYK2 inhibitors (e.g., deucravacitinib) marks a new era in oral immunotherapy, offering improved selectivity and favorable safety profiles. Apremilast, a PDE4 inhibitor, remains an important option for patients with moderate disease or contraindications to biologics. In melasma, randomized controlled trials have validated the efficacy of oral tranexamic acid, though optimal dosing and long-term safety remain under investigation. Novel agents targeting oxidative stress, inflammation, and melanocyte biology are in various stages of clinical development for both conditions.
Current guidelines advocate for individualized therapy in psoriasis, with oral agents indicated for moderate-to-severe disease unresponsive to topical treatments or phototherapy. Methotrexate remains the mainstay due to its efficacy and cost-effectiveness, though monitoring for hepatic, hematologic, and pulmonary toxicity is imperative. Cyclosporine is reserved for short-term use, while acitretin is preferred in pustular and erythrodermic forms. In melasma, guidelines recommend oral tranexamic acid as a second-line adjunct, particularly in recalcitrant cases, with caution regarding thromboembolic risk. Shared decision-making, laboratory surveillance, and patient education are essential components of guideline-based management.
Oral treatment strategies for psoriasis and melasma are rapidly evolving, fueled by advances in pathogenetic understanding and the emergence of novel therapeutic agents. For clinicians, judicious selection and monitoring of oral therapies are critical to maximizing efficacy while minimizing adverse events. As research continues to elucidate new targets and refine existing approaches, the future holds promise for more personalized, mechanism-based oral interventions that can transform outcomes for patients with these challenging chronic dermatologic conditions.
