Intergenerational models emphasize shared care plans, medication reconciliation, fall prevention, chronic disease management, and advance care planning. Polypharmacy reduction, deprescribing, and non-pharmacological interventions such as physical activity, cognitive training, and community engagement are central. Intergenerational involvement ensures adherence, facilitates transitions of care, and supports palliative and end-of-life care coordination, ultimately improving patient outcomes and satisfaction.

Emerging evidence from randomized trials suggests that intergenerational interventions can delay institutionalization, reduce emergency visits, and improve mental health outcomes. Integration of artificial intelligence for risk stratification, remote monitoring, and predictive analytics further enhances the effectiveness of longitudinal care models for older adults.
Professional organizations such as the American Geriatrics Society and the World Health Organization endorse patient-centered, longitudinal, and team-based care for older adults. Intergenerational models are highlighted as best practice for continuity, chronic disease management, and psychosocial well-being.
