Parkinson's disease (PD) is a neurodegenerative disorder that primarily affects dopamine-producing neurons in the substantia nigra, a part of the brain. Its prevalence is increasing, necessitating a comprehensive understanding among healthcare professionals.
The etiology of PD remains largely unknown. Both genetic and environmental factors are implicated, with a small percentage of cases being familial. The pathogenesis involves the loss of dopaminergic neurons and the presence of Lewy bodies, abnormal aggregates of protein in the surviving neurons.
PD typically presents with motor symptoms such as bradykinesia, rigidity, tremor, and postural instability. Non-motor symptoms can precede motor symptoms and include olfactory dysfunction, autonomic dysfunction, sleep disorders, and neuropsychiatric symptoms.
Diagnosis is primarily clinical, based on history and physical examination. There are currently no definitive diagnostic tests, although imaging studies like DaTSCAN can support the diagnosis. The UK Brain Bank Criteria is widely used for diagnosis.
Management of PD is symptomatic, aiming at improving quality of life. Pharmacological therapy includes levodopa, dopamine agonists, and MAO-B inhibitors. Surgical interventions such as deep brain stimulation may be considered in advanced disease.
PD is a progressive disease with variable rate of progression. Factors influencing prognosis include age at onset, severity of symptoms, and response to treatment. Complications can include falls, dysphagia, and cognitive impairment.
Understanding PD is crucial for healthcare professionals as its prevalence increases. Despite the lack of definitive diagnostic tests and cure, symptom management can significantly improve the quality of life of affected individuals. Ongoing research aims to further unravel the mysteries of this complex disease.
