The female reproductive lifespan is characterized by distinct physiological milestones: menarche, fertility, pregnancy, perimenopause, and menopause. Each phase is associated with hormonal, metabolic, and psychosocial shifts that impact overall health and disease risk. A comprehensive, milestone-focused approach enables healthcare professionals to anticipate health needs, implement preventive strategies, and tailor interventions to the evolving clinical landscape of women's lives.
Globally, women experience diverse health challenges at each reproductive milestone. Menstrual disorders affect up to 30% of adolescent girls, with dysmenorrhea and abnormal uterine bleeding being prevalent concerns. During the reproductive years, infertility impacts 8-12% of couples, and pregnancy-related morbidity ranging from gestational diabetes to hypertensive disorders remains a significant contributor to global disease burden. Perimenopause and menopause affect virtually all women, with vasomotor symptoms reported in 60-80% and increased cardiovascular, metabolic, and bone health risks emerging post-menopause. Recent innovations are reshaping care across reproductive milestones. Long-acting reversible contraceptives (LARC) offer highly effective, low-maintenance options for menstrual management and contraception. Advances in ART, including preimplantation genetic testing and oocyte cryopreservation, have improved fertility outcomes. Novel pharmacotherapies for menopausal symptoms such as selective estrogen receptor modulators (SERMs), neurokinin B antagonists, and tissue-selective estrogen complexes provide alternatives for women contraindicated for HRT.
