Parkinson's disease (PD) is a progressive neurodegenerative disorder primarily affecting the motor system. It is characterized by bradykinesia, resting tremor, rigidity, and postural instability. This article aims to provide a comprehensive overview of PD, its pathophysiology, clinical presentation, diagnosis, and management.
The pathophysiology of PD is complex, involving the degeneration of dopamine-producing neurons in the substantia nigra pars compacta. This leads to an imbalance in the dopaminergic and cholinergic systems in the basal ganglia, resulting in the characteristic motor symptoms. The presence of Lewy bodies, protein aggregates primarily composed of alpha-synuclein, is a key neuropathological feature.
PD typically presents with asymmetric onset of motor symptoms. Non-motor symptoms such as autonomic dysfunction, sleep disturbances, and cognitive impairment often precede motor symptoms. Diagnosis is primarily clinical, based on history and examination. Neuroimaging can support the diagnosis but is not routinely required.
Management of PD is multidimensional, involving pharmacological and non-pharmacological interventions. Pharmacotherapy, primarily with levodopa, aims to improve motor symptoms. Non-pharmacological interventions include physiotherapy, occupational therapy, and speech and language therapy. Deep brain stimulation may be considered in advanced cases.
PD is a chronic, progressive disease with variable progression rates. While motor symptoms can be managed effectively for many years, non-motor symptoms and complications of therapy often become the major sources of disability in advanced disease.
Understanding PD is crucial for healthcare professionals as it impacts the quality of life of affected individuals and their caregivers. Early recognition, appropriate management, and regular follow-up can significantly improve the patient's prognosis and quality of life.
