Transitioning to VBHC involves several key steps. Firstly, healthcare providers must adopt a patient-centric approach, focusing on improving health outcomes. Secondly, the measurement of outcomes and costs across the full cycle of care is crucial. Thirdly, integrated practice units (IPUs) should be established to provide comprehensive care.
