Elevated liver enzymes often pose a diagnostic challenge to clinicians due to their multifactorial etiology. Understanding the underlying causes and implementing appropriate management strategies is crucial for patient care.
Liver enzymes, primarily alanine aminotransferase (ALT) and aspartate aminotransferase (AST), are key indicators of hepatic health. Their elevation may indicate hepatocellular injury, cholestasis, or infiltrative diseases. The degree of elevation and the AST:ALT ratio can provide valuable clues towards the underlying pathology.
Common causes include viral hepatitis, alcohol-related liver disease, non-alcoholic fatty liver disease (NAFLD), and drug-induced liver injury. Less common causes encompass autoimmune hepatitis, metabolic disorders like Wilson’s disease and hemochromatosis, and infiltrative diseases such as sarcoidosis and malignancies.
An initial workup should include a thorough history and physical examination, followed by laboratory investigations to assess liver function, screen for viral hepatitis, and evaluate for metabolic and autoimmune disorders. Imaging studies may be required to rule out structural abnormalities. Liver biopsy is reserved for cases where non-invasive tests fail to identify the cause.
Management depends on the underlying cause. Lifestyle modifications and alcohol abstinence are recommended in NAFLD and alcohol-related liver disease, respectively. Antiviral therapy is indicated in viral hepatitis, whereas immunosuppressive therapy may be required in autoimmune hepatitis. Regular monitoring of liver enzymes is essential to assess treatment response and disease progression.
Elevated liver enzymes require a systematic diagnostic approach to identify the underlying cause. A comprehensive understanding of the potential etiologies, along with a tailored management strategy, can lead to improved patient outcomes and prevent progression to advanced liver disease.
