Initial Evaluation

Initial evaluation should involve a thorough history, physical examination, and risk stratification. Important factors include age, gender, exposure to radiation, family history, and the presence of symptoms such as dysphagia, hoarseness, or rapid nodule growth. High-resolution neck ultrasonography is the imaging modality of choice for initial assessment.

Role of Fine-Needle Aspiration Biopsy (FNAB)

FNAB remains the gold standard for differentiating benign from malignant nodules. The decision to perform FNAB should be based on the size and ultrasonographic characteristics of the nodule. The Bethesda System for Reporting Thyroid Cytopathology provides a standardized approach to interpreting FNAB results and guiding management.

Management Strategies

Management of thyroid nodules is guided by the risk of malignancy. Benign nodules may be monitored with periodic ultrasonography. Indeterminate nodules may warrant repeat FNAB, molecular testing, or referral for surgical evaluation. Malignant or highly suspicious nodules generally require surgical intervention.

Thyroid Cancer and Postoperative Surveillance

Patients with thyroid cancer require postoperative surveillance, which involves physical examination, thyroglobulin measurement, and neck ultrasonography. Radioactive iodine ablation may be considered in certain high-risk patients. Long-term follow-up is crucial as recurrence can occur years after initial treatment.
