Liver cirrhosis, a late-stage liver disease, is characterized by fibrosis and the formation of regenerative nodules. It is a significant global health issue with a multitude of complications that require comprehensive management strategies.
Portal hypertension is a frequent complication of cirrhosis, often leading to varices and ascites. Management includes beta-blockers for primary prophylaxis and endoscopic band ligation for secondary prophylaxis of variceal bleeding. Diuretics and sodium restriction are primary treatment options for ascites.
Hepatic encephalopathy (HE), characterized by cognitive impairment and altered consciousness, is another common complication. Lactulose and rifaximin are the mainstays of therapy. Early detection and treatment of precipitating factors, such as infections and gastrointestinal bleeding, are crucial.
Hepatorenal syndrome (HRS) is a severe complication characterized by renal failure in the absence of kidney disease. Albumin and vasoconstrictors, such as terlipressin, are the first-line treatment. Renal replacement therapy and liver transplantation may be required in severe cases.
Hepatocellular carcinoma (HCC) is a leading cause of death in cirrhotic patients. Surveillance with ultrasound and alpha-fetoprotein levels is recommended every six months. Treatment options include surgical resection, liver transplantation, and locoregional therapies.
Managing complications of liver cirrhosis requires a comprehensive, multidisciplinary approach. Early recognition and prompt management of these complications can significantly improve patient outcomes. It is crucial for healthcare professionals to stay updated on the latest therapeutic strategies to provide optimal patient care.
