Vertigo is a subjective sensation of movement, usually rotational, experienced by patients. It is often associated with conditions affecting the vestibular system. Differentiating between peripheral and central causes of vertigo is crucial for appropriate management.
History taking and physical examination form the cornerstone in diagnosing vertigo. Emphasis should be on the onset, duration, and character of vertigo, associated symptoms like hearing loss or tinnitus, and neurological signs. Bedside tests like the Dix-Hallpike maneuver and head impulse test can aid in diagnosis.
Management of vertigo depends on the underlying etiology. Peripheral vertigo can be managed with vestibular rehabilitation and medications like meclizine. For central vertigo, the underlying neurological condition needs to be addressed. Patient education and reassurance also play a pivotal role.
Imaging studies, especially MRI, can be useful in cases where central vertigo is suspected. They help in identifying conditions like cerebellar infarcts or tumors which could present with vertigo.
Recent advancements have led to the development of vestibular implants for patients with refractory vertigo. More research is needed to assess their long-term safety and efficacy.
