Peptic Ulcer Disease (PUD) remains a prevalent health issue with significant morbidity and mortality worldwide. The management of PUD has evolved significantly over the past decades, thanks to advancements in diagnostic tools, pharmacotherapy, and endoscopic interventions.
Modern diagnostic techniques have revolutionized the detection of PUD. The advent of endoscopy has enabled direct visualization of ulcers, while urea breath tests and stool antigen tests have improved the accuracy of Helicobacter pylori detection. Non-invasive serological tests also facilitate the early identification of H. pylori infection, the primary cause of PUD.
Pharmacotherapy for PUD has seen significant advancements. Proton pump inhibitors (PPIs) remain the cornerstone of treatment, providing superior acid suppression and ulcer healing. The development of quadruple therapy, including PPIs, bismuth, and two antibiotics, has increased the eradication rate of H. pylori, reducing ulcer recurrence.
Endoscopic interventions have become an essential part of PUD management, particularly for complications such as bleeding. Techniques such as endoscopic clipping, thermal coagulation, and injection sclerotherapy have improved the control of ulcer bleeding, reducing the need for surgery.
Best practices in PUD management include prompt diagnosis, aggressive H. pylori eradication, and risk factor modification. Patient education on the avoidance of NSAIDs and smoking, along with stress management, can help prevent ulcer recurrence. Regular follow-ups to monitor treatment efficacy and detect complications early are also crucial.
Advancements in diagnostic tools, pharmacotherapy, and endoscopic interventions have considerably improved PUD management. By adhering to best practices, healthcare professionals can enhance patient outcomes, reduce complications, and improve the quality of life for individuals living with PUD.
