Unexplained weight loss, medically termed as involuntary weight loss (IWL), is a decrease in body weight that occurs when a patient is not dieting or trying to lose weight. IWL can be a marker of a serious underlying condition, and its etiology can be a diagnostic challenge for physicians due to its broad differential diagnosis.
Unexplained weight loss can occur due to various reasons, including malignancies, endocrine disorders, gastrointestinal diseases, psychiatric conditions, and infections. The most common causes in the elderly are malignancies, depression, and social factors. In younger patients, HIV infection, endocrine diseases, and malabsorption syndromes are more frequent. However, in about 15-25% of cases, the cause remains unidentified even after thorough evaluation.
A systematic approach to the evaluation of unexplained weight loss involves a comprehensive history, physical examination, and appropriate investigations. The history should focus on the duration and amount of weight loss, dietary intake, medication use, and associated symptoms. The physical examination should focus on signs of systemic diseases. Initial investigations usually include complete blood count, metabolic panel, thyroid function tests, and urinalysis. Further investigations, such as imaging or endoscopy, are guided by clinical suspicion.
The management of unexplained weight loss primarily involves treating the underlying cause. In cases where no cause is identified, empirical treatment may be considered. Nutritional support and dietary modifications can be beneficial irrespective of the underlying cause. Regular follow-up is essential to monitor the patient's progress and response to treatment.
Unexplained weight loss is a complex clinical problem that requires a comprehensive and systematic approach for effective diagnosis and management. Understanding the potential causes and adopting a stepwise evaluation can help in identifying the underlying etiology, thereby guiding appropriate treatment strategies. Further research is needed to enhance our understanding of this challenging clinical scenario and to develop more effective diagnostic and therapeutic interventions.
