Unexplained weight loss, defined as a loss of 5% or more of body weight within a six to twelve-month period, can be a harbinger of serious underlying disease. This article aims to provide a comprehensive approach towards diagnosing the underlying cause.
The first step in evaluating unexplained weight loss is a detailed history and physical examination. This includes an assessment of dietary intake, physical activity levels, psychiatric status, and a comprehensive review of systems. The physical examination should focus on signs of underlying systemic disease.
Initial diagnostic tests should include a complete blood count, metabolic panel, thyroid function tests, urine analysis, and stool tests if indicated. Imaging studies such as chest X-ray, abdominal ultrasound or CT scan may be warranted based on the patient's presenting symptoms and signs. Endoscopy and colonoscopy may be considered in patients above 50 years of age or in those with alarming symptoms.
Special considerations should be given to elderly patients, as weight loss in this population can be associated with malignancy, depression, or social isolation. In younger patients, eating disorders and malabsorption syndromes should be considered.
Management should be targeted towards the underlying cause. If no cause is identified after initial evaluation and testing, close follow-up is recommended. A stepwise approach to further testing should be considered, to avoid unnecessary investigations and to minimize patient distress.
In conclusion, unexplained weight loss is a complex clinical problem that requires a systematic approach. A thorough history and physical examination, coupled with appropriate diagnostic testing, can help identify the underlying cause and guide management.
