Peptic ulcer disease (PUD) continues to be a significant global health challenge, with a substantial impact on both patient quality of life and healthcare costs. However, recent advancements in diagnostic methods and treatment strategies have revolutionized the management of this disease.
The gold standard for diagnosing PUD is endoscopy, which allows direct visualization of the ulcer. However, non-invasive tests such as the urea breath test (UBT) and stool antigen test (SAT) for Helicobacter pylori, the most common cause of PUD, have become increasingly popular. These tests are simple, safe, and highly accurate, making them ideal for initial screening and post-treatment eradication confirmation.
Advancements in the treatment of PUD have mainly focused on eradicating H. pylori and reducing gastric acid secretion. Triple therapy, consisting of a proton pump inhibitor (PPI) and two antibiotics, is the most commonly used regimen. However, the emergence of antibiotic-resistant H. pylori strains has led to the development of quadruple therapy, which includes a PPI, bismuth, and two antibiotics. Furthermore, the use of PPIs has been optimized to provide maximal acid suppression, thereby promoting ulcer healing and reducing complications.
Despite these advancements, the management of PUD remains challenging due to increasing antibiotic resistance and the adverse effects of long-term PPI use. Therefore, future research should focus on developing new antibiotics, optimizing PPI use, and exploring alternative therapeutic strategies such as probiotics and vaccines.
Advancements in the diagnosis and treatment of PUD have significantly improved patient outcomes. However, ongoing research is needed to tackle the challenges posed by antibiotic resistance and long-term PPI use. As healthcare professionals, staying abreast of these advancements is crucial to provide optimal care to patients with PUD.
