Chronic cough, defined as a persistent cough lasting more than eight weeks, is a prevalent yet challenging condition to diagnose and manage. It can significantly impair patients' quality of life and often presents a diagnostic enigma to clinicians due to its multifactorial etiology.
Common causes include gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS). However, the cough may also be a symptom of less common conditions like lung cancer or sarcoidosis. A thorough history and physical examination, supplemented by targeted investigations based on clinical suspicion, form the cornerstone of diagnosis.
Treatment of chronic cough is primarily aimed at addressing the underlying cause. Empirical treatment for the most common causes (GERD, UACS, and asthma) may be a pragmatic initial approach. However, refractory cases require a more comprehensive evaluation, including imaging studies and pulmonary function tests. In some cases, referral to a specialist may be necessary.
A multidisciplinary approach involving allergists, pulmonologists, gastroenterologists, and even speech-language pathologists can be beneficial in managing complex cases. This approach ensures comprehensive care, addressing not only the cough but also associated conditions like laryngeal hypersensitivity and swallowing disorders.
Emerging research on neuronal pathways and cough reflex sensitivity may yield novel therapeutic targets. In addition, the role of behavioral therapies in managing chronic cough is being increasingly recognized, opening new avenues for non-pharmacological interventions.
Chronic cough is a complex condition requiring a comprehensive and individualized approach to diagnosis and management. A thorough understanding of the etiological factors, coupled with a multidisciplinary approach, can significantly improve patient outcomes. Future research holds promise for more effective and targeted treatments.
