Chronic cough, defined as a cough persisting for more than eight weeks, is a frequent reason for medical consultations. Despite its prevalence, the management of chronic cough remains a clinical challenge due to its multifactorial etiology and complex pathophysiology.
Chronic cough can be viewed as a hypersensitive state of the cough reflex arc. This hypersensitivity can be triggered by numerous conditions such as gastroesophageal reflux disease (GERD), upper airway cough syndrome (UACS), and eosinophilic bronchitis. Understanding the underlying pathophysiology is critical for appropriate management.
An effective diagnostic approach begins with a comprehensive history and physical examination. The use of validated questionnaires and objective measures of cough severity can be helpful. Diagnostic tests should be guided by clinical suspicion. Empirical treatment trials for the most common causes of chronic cough (GERD, UACS, and asthma) can also be considered when the diagnosis is uncertain.
Management should be tailored to the underlying cause. Acid-suppressive therapy for GERD, inhaled corticosteroids for asthma, and antihistamines for UACS are some of the commonly used treatments. Non-pharmacological interventions, including speech and physical therapy, can also be beneficial. For refractory cases, novel therapies targeting the cough reflex pathway are being explored.
Despite these strategies, a significant proportion of patients continue to have a persistent cough. This highlights the need for further research to understand the mechanisms of chronic cough and to develop more effective treatments. Emerging data on neuronal modulation and genetic influences on cough reflex sensitivity offer promising avenues for future investigation.
Chronic cough is a complex clinical entity requiring a comprehensive diagnostic and therapeutic approach. While progress has been made, there remain significant gaps in our understanding and management of this condition. Continued research and clinical innovation are crucial to improving outcomes for patients with chronic cough.
