Gastrointestinal bleeding (GIB) is a common medical emergency that healthcare professionals encounter. Understanding its complex pathophysiology and mastering its clinical approach are pivotal in improving patient outcomes.
Upper GIB is most commonly due to peptic ulcer disease, followed by esophageal varices and gastritis. Lower GIB, on the other hand, is often caused by diverticulosis, angiodysplasia, and colorectal neoplasms. The pathophysiology of GIB involves the disruption of the mucosal barrier, leading to exposure of subepithelial blood vessels to gastric acid and pepsin.
Patients with GIB often present with hematemesis, melena, or hematochezia. However, the clinical presentation may vary, and some patients may present with non-specific symptoms such as fatigue, dizziness, or syncope.
The diagnostic approach to GIB involves both clinical assessment and investigations. Clinical assessment includes a thorough history and physical examination, while investigations may involve laboratory tests, endoscopy, and imaging studies.
Management of GIB involves resuscitation, risk stratification, and definitive treatment. Resuscitation includes fluid resuscitation and blood transfusion if necessary. Risk stratification helps to determine the need for hospital admission and the urgency of endoscopy. Definitive treatment depends on the cause of the bleed and may involve pharmacological therapy, endoscopic therapy, or surgery.
Mastering the clinical approach to GIB is crucial for healthcare professionals. This involves understanding the etiology and pathophysiology, recognizing the clinical presentation, knowing the diagnostic approach, and being familiar with the principles of management. With this knowledge, healthcare professionals can provide optimal care for patients with GIB and improve patient outcomes.
