Unexplained elevations in liver enzymes often pose a diagnostic challenge for clinicians. A systematic approach to evaluation and management can help determine the underlying cause and guide appropriate treatment.
Initial evaluation should include a comprehensive history and physical examination. Key elements to consider are alcohol consumption, medication use, family history of liver disease, and risk factors for viral hepatitis. Laboratory evaluation should include a complete blood count, coagulation studies, and serologic tests for viral hepatitis.
The differential diagnosis for elevated liver enzymes is broad. Common causes include alcoholic liver disease, nonalcoholic fatty liver disease, viral hepatitis, and drug-induced liver injury. Less common causes include autoimmune hepatitis, metabolic liver diseases, and infiltrative diseases such as hemochromatosis or Wilson's disease.
Management depends on the underlying cause. For alcoholic liver disease, the mainstay of treatment is abstinence from alcohol. Nonalcoholic fatty liver disease is managed primarily through lifestyle modifications such as weight loss and exercise. Viral hepatitis requires antiviral therapy, while drug-induced liver injury necessitates discontinuation of the offending drug.
Following the initiation of treatment, patients should be closely monitored for improvement in liver enzyme levels. In cases where the cause is not identified or liver enzymes remain elevated despite treatment, referral to a hepatologist for further evaluation may be warranted.
Elevated liver enzymes are a common clinical problem that requires a systematic approach to evaluation and management. By understanding the common causes and appropriate management strategies, clinicians can effectively diagnose and treat patients with liver enzyme abnormalities.
