Pyrexia, commonly known as fever, is a prevalent symptom in clinical practice. It signifies an underlying pathophysiological process, often an infection or inflammation. Understanding its mechanisms, diagnostic strategies, and management principles is crucial for effective patient care.
Fever is a complex response to pyrogens that reset the hypothalamic thermoregulatory center. Pyrogens can be exogenous, like microbial toxins, or endogenous, such as interleukin-1 (IL-1), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-alpha). These cytokines trigger prostaglandin E2 (PGE2) production, leading to a rise in the set-point temperature.
Thorough history and physical examination form the cornerstone of fever diagnosis. History should include recent travel, exposures, medications, and immunization status. Fever patterns can provide clues to the underlying cause. Laboratory investigations should be guided by clinical suspicion. In cases of fever of unknown origin (FUO), advanced imaging and invasive procedures might be necessary.
Fever management should be tailored to the patient's condition and the underlying cause. Antipyretics, such as acetaminophen and nonsteroidal anti-inflammatory drugs (NSAIDs), are commonly used. However, they should be used judiciously, considering their side effects. In some cases, fever can serve as a beneficial host response, and its suppression may not always be desirable.
In pediatric and geriatric populations, fever can present differently and can have different implications. In children, fever is often higher and can cause febrile seizures. In the elderly, fever response may be blunted, making diagnosis challenging. These populations may require a more cautious approach.
Pyrexia is a common but complex clinical phenomenon. A systematic approach to its diagnosis and management, considering the patient's individual circumstances and the potential benefits and harms of fever suppression, is necessary for optimal patient care.
