It is critical to distinguish hypertensive emergencies from hypertensive urgencies (severe hypertension without progressive target organ damage), as the management strategies differ significantly.

Managing Hypertensive Emergencies

Immediate reduction of blood pressure is necessary in hypertensive emergencies to prevent or limit further organ damage. However, overly aggressive reduction must be avoided as it can lead to a precipitous drop in perfusion pressure and resultant ischemia. Intravenous antihypertensive medications are the preferred treatment options, with the choice of drug depending largely on the specific organs involved.

Preventing Severe Hypertension

Prevention of severe hypertension primarily involves proper management of chronic hypertension. This includes lifestyle modifications, pharmacotherapy, and regular monitoring of blood pressure. Early detection and management of hypertensive urgencies can also prevent progression to hypertensive emergencies.
