As healthcare professionals, one of the common complaints we encounter is dizziness, a symptom that can be indicative of various underlying conditions. The most prevalent of these is vertigo, a sensation of movement or spinning when one is stationary. This article aims to provide a comprehensive clinical approach to vertigo, helping you identify and manage this condition effectively.
Vertigo is often a symptom of vestibular disorders and is characterized by a sensation of spinning or feeling off-balance. It is primarily attributed to issues in the inner ear or brain. Distinguishing vertigo from other types of dizziness is crucial, as it helps narrow down potential causes and dictates the course of treatment.
Diagnosing vertigo involves a thorough patient history, physical examination, and targeted investigations. Key elements of the history include onset, duration, triggers, and associated symptoms. Physical examination often focuses on nystagmus, head impulse testing, and Romberg's sign. Investigations like electronystagmography (ENG), videonystagmography (VNG), and magnetic resonance imaging (MRI) can be useful in complex cases.
Management of vertigo primarily involves treating the underlying cause. Vestibular suppressants, such as meclizine and diazepam, can provide symptomatic relief but are not recommended for long-term use. Vestibular rehabilitation therapy (VRT), a form of physical therapy, is often beneficial in chronic cases. In some instances, surgical intervention may be necessary.
Vertigo presents a unique challenge due to its varied causes and nonspecific symptomatology. A comprehensive clinical approach, involving careful history taking, physical examination, and appropriate investigations, is crucial to accurately diagnose and manage this condition. As healthcare professionals, it is our responsibility to stay updated on the latest diagnostic techniques and treatment modalities to provide the best care possible for our patients.
