Unexplained weight loss (UWL) is a common yet perplexing clinical presentation, often indicative of underlying health issues. A systematic approach to evaluating UWL can aid in early detection and intervention, improving patient outcomes.
Initial evaluation of UWL should involve a comprehensive history and physical examination. Detailed questioning about dietary habits, lifestyle changes, medications and psychosocial factors may provide valuable clues. Physical examination should focus on signs of malnutrition, malignancy or systemic disease.
Initial laboratory investigations should include complete blood count, metabolic panel, liver function tests, thyroid function tests, and urinalysis. Imaging studies such as chest X-ray, abdominal ultrasound or CT scan may be warranted based on clinical suspicion.
Endocrine disorders, gastrointestinal diseases, malignancies, psychiatric disorders and infections are common causes of UWL. In elderly patients, non-malignant diseases are more common, while in younger patients, psychiatric disorders such as depression and eating disorders should be considered.
Management should be directed towards the underlying cause, if identified. In cases where no cause is found, symptomatic treatment and close monitoring for the development of new symptoms or signs is advised. Nutritional support and counselling may be beneficial in all cases.
Unexplained weight loss remains a diagnostic challenge in clinical practice. A comprehensive and systematic approach to evaluation, encompassing a thorough history, physical examination, and appropriate laboratory and imaging studies, can help identify the underlying cause. Management should be tailored to the specific disease entity, with an emphasis on nutritional support and close monitoring.
