Acute chest pain (ACP) is a common symptom that often poses a diagnostic challenge to clinicians due to its myriad etiologies. A thorough understanding of the differential diagnoses and an organized approach to evaluation are key to successful management.
ACP can stem from a range of conditions, from benign musculoskeletal disorders to life-threatening cardiovascular diseases. The most feared diagnosis, acute coronary syndrome (ACS), is responsible for a significant proportion of ACP presentations. However, gastrointestinal, pulmonary, and psychological conditions can mimic ACS, complicating the diagnostic process.
Given the broad differential, a systematic approach is essential. Clinicians should begin with a detailed history and physical examination, focusing on the characteristics of the pain, associated symptoms, and risk factors. Tools such as the HEART score can aid risk stratification. Subsequently, diagnostic tests such as electrocardiogram (ECG) and cardiac biomarkers should be utilized. In cases where diagnosis remains uncertain, advanced imaging or specialist consultation may be warranted.
The management of ACP should be tailored to the underlying cause. ACS requires urgent intervention, often involving revascularization and antiplatelet therapy. Conversely, non-cardiac causes of ACP may be managed with symptom control and addressing the primary condition. In all cases, patient education and reassurance play a crucial role.
ACP is a complex clinical presentation requiring a comprehensive and systematic approach for effective diagnosis and management. By understanding the wide range of possible etiologies, employing a methodical diagnostic process, and tailoring treatment to the underlying cause, clinicians can ensure optimal patient outcomes in this challenging area of practice.
