Introduction

Tuberculosis (TB) remains a significant global health concern, necessitating advancements in diagnostic approaches to facilitate early detection and treatment. This article provides an overview of contemporary diagnostic tools that have revolutionized TB detection.

GeneXpert MTB/RIF

The World Health Organization endorsed GeneXpert MTB/RIF in 2010 as a frontline test for TB. It utilizes real-time PCR technology to identify Mycobacterium tuberculosis complex (MTBC) and resistance to rifampicin within two hours. Its high sensitivity and specificity have significantly improved TB detection, particularly in HIV co-infected patients and rifampicin-resistant TB cases.

Line Probe Assays

Line Probe Assays (LPAs) are molecular techniques that detect MTBC and resistance to first and second-line drugs. The WHO approved LPAs, including GenoType MTBDRplus and MTBDRsl, for rapid detection of multi-drug resistant TB. These assays provide results within 48 hours, significantly reducing the time to diagnosis compared to conventional culture methods.

Next-Generation Sequencing

Next-Generation Sequencing (NGS) is a promising tool for TB diagnosis, offering comprehensive insights into the genomic structure of MTBC. NGS can detect drug resistance mutations and provide epidemiological data, aiding in the control of TB outbreaks. However, its application in resource-limited settings is currently limited due to high costs and technical requirements.

Interferon-Gamma Release Assays

Interferon-Gamma Release Assays (IGRAs) are blood tests that measure the immune response to TB antigens. While they cannot differentiate between active and latent TB, IGRAs have proven useful in diagnosing TB infection in individuals with a negative skin test or BCG vaccination.
