Pulmonary hypertension (PH) is a complex and progressive condition characterized by elevated pulmonary artery pressure. It is a significant cause of morbidity and mortality, necessitating a comprehensive understanding for effective management.

Pathophysiology of PH

PH is classified into five groups based on its etiology and pathophysiology. The common thread across all types is the persistent elevation of mean pulmonary arterial pressure, leading to right heart failure. The pathogenesis involves three key processes: vasoconstriction, vascular remodeling, and thrombosis.

Clinical Presentation and Diagnosis

PH often presents non-specifically, with symptoms such as dyspnea, fatigue, and chest pain. Diagnosis requires a high index of suspicion, supported by echocardiography and confirmed by right heart catheterization. Biomarkers, such as N-terminal pro-brain natriuretic peptide, can be useful adjuncts.

Management Strategies

Treatment aims to alleviate symptoms, slow disease progression, and improve quality of life. General measures include diuretics for right heart failure and anticoagulation for thromboembolic disease. Specific therapies target the three pathogenic processes and are tailored to the PH group. Lung transplantation remains the last resort.

Prognosis and Future Directions

Despite advancements, PH prognosis remains poor, with median survival rates of 2.8 years without treatment. Future research focuses on early detection, novel therapeutic targets, and improving patient outcomes through personalized medicine.
