Gastrointestinal bleeding (GIB) is a critical condition that requires swift identification and management. This guide aims to provide healthcare professionals with a comprehensive understanding of the clinical approach to GIB.
Accurate localization of the bleeding source is crucial. Upper GIB is often associated with peptic ulcer disease and gastritis, while lower GIB is commonly linked to diverticulosis and vascular ectasia. Endoscopy is the gold standard diagnostic tool for both upper and lower GIB.
Therapeutic endoscopy remains the mainstay treatment for GIB. It includes thermal coagulation, injection therapy, and mechanical methods. For refractory bleeding, interventional radiology or surgery may be necessary. Pharmacological therapy, including proton pump inhibitors and somatostatin analogues, can also be beneficial.
Initial assessment should focus on hemodynamic stability and resuscitation if required. Risk stratification using validated tools like the Glasgow-Blatchford score can guide management. Patients at high risk should be hospitalized, while those at low risk may be managed as outpatients.
Secondary prevention is vital in GIB management. This includes the eradication of Helicobacter pylori in peptic ulcer disease, use of non-aspirin non-steroidal anti-inflammatory drugs (NSAIDs), and careful management of anticoagulation therapy.
In conclusion, mastering the clinical approach to GIB involves accurate identification of the bleeding source, appropriate use of treatment modalities, efficient management of patients, and effective implementation of secondary prevention strategies. By adopting a comprehensive approach, healthcare professionals can significantly improve patient outcomes in GIB.
