Peptic ulcer disease (PUD) is a prevalent gastrointestinal disorder characterized by the erosion of the gastric or duodenal mucosa. Despite the advances in understanding its etiology and treatment, managing PUD remains a clinical challenge due to its multifactorial nature and the increasing prevalence of drug-resistant strains of Helicobacter pylori.
The pathogenesis of PUD involves an imbalance between aggressive factors such as gastric acid, pepsin, and H. pylori, and defensive factors like bicarbonate, prostaglandins, and mucus. H. pylori infection plays a pivotal role, with its virulence factors disrupting the mucosal defense and promoting inflammation.
Diagnosis of PUD includes a combination of clinical history, physical examination, and confirmatory tests. Endoscopy is the gold standard for diagnosis, providing direct visualization of the ulcer and facilitating biopsy for H. pylori detection. Non-invasive tests such as urea breath test and stool antigen test are also useful.
Management of PUD primarily focuses on eradicating H. pylori and reducing gastric acidity. The standard regimen includes a proton pump inhibitor and two antibiotics for 10-14 days. However, the rising rates of antibiotic resistance necessitate the use of quadruple therapy or tailored regimens based on local resistance patterns.
Alongside pharmacological treatment, lifestyle modifications play a crucial role in PUD management. These include smoking cessation, limiting alcohol and NSAID use, and dietary changes. Although no specific diet can prevent or cure PUD, a balanced diet can help maintain overall health and enhance drug efficacy.
Peptic ulcer disease management requires a comprehensive approach that encompasses an understanding of the complex pathophysiology, appropriate diagnostic strategies, effective therapeutic regimens, and lifestyle modifications. Continued research and education are essential to optimize patient outcomes and tackle the challenges posed by antibiotic resistance.
