Anemia, a condition characterized by a decrease in the total number of red blood cells (RBCs) or hemoglobin, is a common clinical problem encountered by healthcare professionals. With a myriad of potential causes, anemia presents a diagnostic challenge requiring a systematic and comprehensive approach.
Anemia can be classified based on the morphological characteristics of RBCs or the pathophysiological processes involved. Microcytic, normocytic, and macrocytic anemias, each imply distinct etiologies, with iron deficiency, anemia of chronic disease, and vitamin B12 or folate deficiency, respectively. Pathophysiological classification includes production defects (hypoproliferative) and increased loss or destruction (hyperproliferative) of RBCs.
A thorough history and physical examination is the first step in the diagnostic workup. This should be followed by initial laboratory tests including complete blood count, reticulocyte count, and peripheral smear. Reticulocyte count helps distinguish between hypoproliferative and hyperproliferative anemias, while peripheral smear can provide clues on etiology. Further diagnostic tests should be guided by these initial results and clinical suspicion.
In situations where initial investigations are inconclusive, special tests may be necessary. These include serum iron studies, vitamin B12 and folate levels, hemoglobin electrophoresis, bone marrow examination, and tests for hemolysis. It is important to remember that these tests should be ordered judiciously, considering their cost and invasiveness.
While anemia is a common clinical problem, its causes are diverse and often complex. A systematic approach to diagnosis, beginning with a thorough history and physical examination, followed by appropriate laboratory tests, can help unravel the complexity of this condition. Special investigations should be reserved for cases where initial tests are inconclusive. This comprehensive diagnostic approach will enable healthcare professionals to accurately diagnose and effectively manage patients with anemia.
