Acute chest pain (ACP) is a common and challenging presentation in clinical practice, often signifying severe conditions such as acute coronary syndrome (ACS). However, the differential diagnosis is broad, encompassing less critical conditions, making its evaluation complex and crucial.
Initial evaluation should include a comprehensive history, physical examination, and initial diagnostic tests. The history should focus on pain characteristics and associated symptoms. Risk factors for ACS should be identified. Physical examination, although often normal in ACS, can reveal important clues in other conditions. Initial diagnostic tests should include an electrocardiogram (ECG) and cardiac biomarkers.
Clinical decision rules like the HEART score can aid risk stratification and guide management. The HEART score incorporates history, ECG, age, risk factors, and troponin level, categorizing patients into low, intermediate, and high-risk groups. It has demonstrated high sensitivity and specificity in predicting major adverse cardiac events.
For patients with inconclusive initial evaluation, advanced diagnostic tests may be necessary. These include stress testing, coronary computed tomography angiography (CCTA), and invasive coronary angiography. The choice of test depends on the patient's pretest probability of ACS, risk stratification, and local resources.
Management strategies depend on the underlying cause. ACS requires urgent revascularization and medical therapy. Other conditions may need specific treatments or reassurance. All patients should receive risk factor modification and follow-up arrangements.
Evaluating and managing ACP requires a systematic and comprehensive approach. Clinical decision rules can aid in this process. Advanced diagnostic tests may be necessary in certain scenarios. Ultimately, the goal is to accurately diagnose the underlying cause and initiate appropriate management, while minimizing unnecessary investigations and potential harm.
