Chronic cough, defined as a cough that persists for over eight weeks, is a common yet perplexing clinical scenario. Not only does it cause significant patient discomfort, but its etiology can be multifactorial and difficult to ascertain. This article aims to provide a comprehensive approach to the evaluation and management of chronic cough.
Chronic cough can be a symptom of an underlying systemic disease or a primary disorder of the cough reflex. The most common causes include asthma, gastroesophageal reflux disease (GERD), and upper airway cough syndrome (UACS). However, the mechanism may also be idiopathic, or the result of a hypersensitive cough reflex.
A thorough history and physical examination are crucial in the initial evaluation. This should be followed by a structured diagnostic approach, beginning with common causes and progressing to more rare conditions if necessary. Empiric treatment trials for asthma, GERD, and UACS can be considered. If the cough persists, further testing such as chest imaging, spirometry, or bronchoscopy may be warranted.
Treatment of chronic cough should aim to address the underlying cause. This may involve pharmacologic therapies such as bronchodilators for asthma, proton pump inhibitors for GERD, or antihistamines for UACS. In refractory cases, non-pharmacologic therapies such as speech therapy or behavioral interventions can be beneficial. Furthermore, recent advances in understanding the neurophysiology of cough have led to the development of novel cough suppressants targeting specific neural pathways.
Chronic cough is a complex clinical problem that requires a comprehensive approach to diagnosis and treatment. By understanding the underlying mechanisms and adopting a structured diagnostic approach, healthcare professionals can more effectively manage this challenging condition. Continued research into the neurophysiology of cough will likely yield further advances in treatment strategies.
