Pulmonary function testing (PFT) is a crucial tool in diagnosing and managing respiratory diseases. However, understanding the results can be complex. This guide aims to aid healthcare professionals in interpreting PFT results accurately.
Spirometry measures the volume and speed of air that can be inhaled and exhaled. Key parameters include Forced Vital Capacity (FVC), Forced Expiratory Volume in one second (FEV1), and their ratio (FEV1/FVC). An FEV1/FVC ratio less than 0.7 suggests obstructive lung disease, while a lower than predicted FVC suggests restrictive disease.
DLCO measures the lung’s ability to transfer gas from air in the lung to red blood cells in lung blood vessels. A decreased DLCO can indicate emphysema, pulmonary fibrosis, or pulmonary hypertension, among other conditions.
Body plethysmography measures total lung capacity (TLC). A decreased TLC can confirm restrictive lung disease, while an increased TLC can indicate hyperinflation seen in diseases like emphysema.
Flow-volume loops graphically represent inhalation and exhalation flows against volumes. The shape of the loop can suggest specific diagnoses. For example, a "scooped out" expiratory loop suggests obstructive disease, while a truncated inspiratory loop suggests fixed upper airway obstruction.
This test is used to diagnose asthma. A positive test, indicated by a 20% drop in FEV1 following methacholine inhalation, confirms the diagnosis.
PFT interpretation is a complex but essential skill for healthcare professionals managing patients with respiratory diseases. Understanding the key components of PFTs and their implications can greatly enhance patient care.
