Tuberculosis (TB) remains a significant global health issue, necessitating innovative diagnostic techniques for its eradication. This article provides a comprehensive review of the advancements in TB detection, focusing on contemporary diagnostic approaches.
Introduced in 2010, the GeneXpert MTB/RIF assay revolutionized TB diagnostics by enabling simultaneous detection of Mycobacterium tuberculosis complex (MTBC) and rifampicin resistance within two hours. This nucleic acid amplification test (NAAT) has high sensitivity and specificity, and is endorsed by the World Health Organization (WHO) for TB diagnosis.
Line Probe Assays (LPAs) are another class of NAATs that detect MTBC and resistance to rifampicin and isoniazid. LPAs provide results within 48 hours and are particularly useful in diagnosing multi-drug resistant TB (MDR-TB).
Next-Generation Sequencing (NGS) is a promising tool for TB detection. NGS can sequence the entire MTBC genome, providing comprehensive information about drug resistance and genetic diversity. However, its implementation in routine diagnostics is presently limited due to high costs and technical complexities.
Immunological tests, such as Interferon Gamma Release Assays (IGRAs), are used for latent TB infection detection. These tests measure the immune response to TB-specific antigens, offering an advantage over traditional tuberculin skin tests.
Advanced imaging techniques, including Positron Emission Tomography-Computed Tomography (PET-CT), provide detailed insights into TB pathogenesis and disease progression. These non-invasive techniques can detect TB in extrapulmonary sites, which are often missed by conventional diagnostics.
Contemporary diagnostic approaches have significantly improved TB detection, facilitating early intervention and treatment. However, challenges such as cost, accessibility, and technical complexity persist. Continued research and innovation in TB diagnostics are crucial for global TB control and eradication.
