Acute chest pain is a common symptom that brings patients to the emergency department. Despite being a common presentation, it remains a complex clinical challenge due to its diverse etiology, which ranges from benign conditions to life-threatening diseases such as acute coronary syndrome (ACS), pulmonary embolism, and aortic dissection. The diagnostic approach to acute chest pain requires a thorough understanding of the clinical presentation, risk factors, and diagnostic testing. This article aims to provide a comprehensive approach to diagnosing acute chest pain in clinical practice.
Acute chest pain can present with varying characteristics that can give clues to its etiology. The clinician should obtain a detailed history, focusing on the nature, location, radiation, associated symptoms, and precipitating factors of the pain. It is also important to consider the patient's age, sex, and existing comorbidities as these can influence the likelihood of certain diagnoses.
Recognizing the risk factors for life-threatening causes of chest pain is crucial in the diagnostic process. These include age above 65, male sex, smoking, hypertension, hyperlipidemia, diabetes, family history of premature coronary artery disease, and known coronary artery disease. A thorough risk assessment can guide further diagnostic testing and management decisions.
Diagnostic testing for acute chest pain includes electrocardiogram (ECG), chest radiograph, and cardiac biomarkers. ECG is the first-line investigation and can reveal findings suggestive of ACS, such as ST-segment elevation or T wave inversion. Chest radiograph can help identify non-cardiac causes of chest pain, such as pneumonia or pneumothorax. Cardiac biomarkers, particularly troponins, are highly sensitive and specific for myocardial injury and are essential in diagnosing ACS.
Clinical decision rules, such as the HEART score and the GRACE score, have been developed to aid in the risk stratification and management of patients with acute chest pain. These tools incorporate clinical variables, ECG findings, and cardiac biomarkers to predict the likelihood of major adverse cardiac events. They have been validated in multiple studies and can help guide decision-making in this complex clinical scenario.
Diagnosing acute chest pain requires a systematic approach that includes a thorough history, physical examination, risk assessment, and appropriate diagnostic testing. Clinical decision rules can assist in risk stratification and guide management decisions. A high index of suspicion for life-threatening causes is always necessary, given the potential for catastrophic outcomes. Continued research and education are needed to further refine our diagnostic approach to this common but challenging clinical presentation.
