Pulmonary hypertension (PH) is a condition characterized by elevated pulmonary arterial pressure and secondary right ventricular failure. It is a progressive disorder with a high morbidity and mortality rate, necessitating a comprehensive understanding for effective management.
PH affects individuals of all ages, with a higher prevalence in females. The pathophysiology of PH is complex and multifactorial, involving endothelial dysfunction, vasoconstriction, and vascular remodeling. These changes lead to increased pulmonary vascular resistance, ultimately resulting in right ventricular overload and failure.
PH often presents insidiously, with nonspecific symptoms such as dyspnea, fatigue, and syncope. As the disease progresses, patients may develop signs of right heart failure, including peripheral edema and ascites. The diagnosis is often delayed due to the nonspecific nature of these symptoms.
The diagnostic approach to PH involves a combination of clinical evaluation, echocardiography, and right heart catheterization. Echocardiography is often the initial test of choice, providing an estimate of pulmonary artery pressure and assessing right ventricular function. Right heart catheterization, however, remains the gold standard for diagnosis, allowing for direct measurement of pulmonary artery pressure and pulmonary vascular resistance.
Management of PH involves a multipronged approach, including general measures, targeted therapies, and in severe cases, lung transplantation. General measures include anticoagulation, diuretics, and oxygen therapy. Targeted therapies aim to reverse the pathophysiological changes in PH, and include endothelin receptor antagonists, phosphodiesterase-5 inhibitors, and prostacyclin analogs.
PH is a complex, progressive disorder that requires a comprehensive understanding for effective management. Early recognition, accurate diagnosis, and appropriate treatment strategies can significantly improve patient outcomes. As healthcare professionals, staying informed about the latest advancements in PH is crucial to delivering optimal patient care.
