Osteoporosis, a systemic skeletal disease characterized by low bone mass and microarchitectural deterioration, poses a significant public health issue. With the aging population, the prevalence of osteoporosis and associated fractures is on the rise. This necessitates the need for advanced management strategies.
High-resolution peripheral quantitative computed tomography (HR-pQCT) and trabecular bone score (TBS) are emerging diagnostic tools. They provide detailed insights into bone quality and strength, improving fracture risk prediction beyond bone mineral density (BMD) measurements.
Newer pharmacological options, such as Romosozumab, an anti-sclerostin monoclonal antibody, and Abaloparatide, a parathyroid hormone-related protein analog, have shown promising results in increasing BMD and reducing fracture risk. These drugs offer alternatives to bisphosphonates, especially for patients with contraindications or intolerances.
Emerging evidence supports the role of high-intensity resistance and impact training (HiRIT) in improving BMD and functional performance. Nutritional interventions, particularly adequate calcium and vitamin D intake, remain indispensable.
There is a growing emphasis on personalized treatment. This involves individual risk assessment, considering factors such as age, sex, fracture history, and comorbidities, to tailor the most effective and safe treatment plan.
Advancements in osteoporosis management are revolutionizing patient care, with novel diagnostic tools, pharmacological agents, and personalized treatment strategies. Continued research and clinical trials are crucial to further refine these approaches and improve outcomes for osteoporosis patients.
