Acute chest pain (ACP) is one of the most common reasons for emergency department visits globally. It poses a diagnostic challenge due to its diverse etiologies, which range from benign musculoskeletal disorders to life-threatening conditions like acute coronary syndrome (ACS). This article aims to provide a comprehensive approach to the evaluation and management of ACP.
Initial assessment should focus on patient history, physical examination, and electrocardiogram (ECG). These can provide crucial clues about the underlying cause. History should include the PQRST (Provocation, Quality, Radiation, Severity, Timing) of pain, associated symptoms, and risk factors for coronary artery disease. An ECG should be obtained within 10 minutes of presentation.
Cardiac biomarkers, particularly troponins, play a key role in the diagnosis of ACS. High-sensitivity troponin assays have improved the early detection of myocardial injury. However, elevated troponins can also be seen in non-cardiac conditions, necessitating careful interpretation.
When initial evaluation is inconclusive, imaging studies can be invaluable. Chest radiography, echocardiography, and computed tomography (CT) angiography are commonly used. CT angiography is particularly useful in evaluating for pulmonary embolism and aortic dissection.
Risk stratification helps guide management. Patients at low risk for ACS may undergo outpatient evaluation, while those at high risk require hospital admission. Various risk scores like the HEART score can assist in this process.
The approach to ACP involves a combination of clinical assessment, biomarker testing, imaging studies, and risk stratification. A systematic and comprehensive approach can help ensure that life-threatening causes are not missed, while also avoiding unnecessary hospital admissions for low-risk patients. Continuing research and advancements in diagnostic modalities promise to further refine this approach in the future.
